***NOTE:

1. You must bring request for "Live Scan Fingerprint Background Check Request" form with you to your appointment and a valid picture ID.

2. It is very important that you have the correct form.  Neither Mosley's Mobile Notary,  the DOJ nor the FBI will issue you a refund. 

3. If you are not sure whether or not you have the correct form, please contact whomever is requiring you to be fingerprinted and have

them direct you to the proper form before your appointment.  No refund will be issued. 

 4. Live Scan Fingerprints are transmission to Michigan DOJ & FBI for Background Check.
